New Students - How to Register
Admission is based on openings in each class. Acceptance cannot be determined without all the appropriate paperwork completed.
New students must have on file: an application, registration form, and tuition payment preference form.

Health Information
All newly registered students entering MCS are required to submit the RI SCHOOL PHYSICAL FORM. The physical must have taken place within the last year. In addition, all students must submit a Student Health History Form.
Medication Policy:
All medication should be supplied by the parent/guardian. Medication Forms are to be submitted prior to the dispensing of any medications in school. Prescription medication requires the signature of the doctor and parent or guardian. Non prescription medication requires the parent/guardian signature.
Pre-K for 2009-2010
Pre-K 3 Day: M/W/F Child must be 3 years of age by September 1, 2009.
Pre-K 5 Day: Child must be 4 years of age by September 1, 2009.
School Uniforms:
Donnelly's School Apparel will be at the school on Wednesday, May 6, 2009 from 9-11 am for measurements. If you can not attend, you can go to the retail store in Providence or order on-line at www.donnellysclothing.com
Extended Day:
An extended day option is available to parents on a daily basis or as needed. This program begins at 7:30 am and ends at 5:30 pm. The cost is additional and separate from tuition.
