Dear Parents/Guardians:

It is that time of the year when we reflect on our mission which is to serve God by providing a quality education in a faith-filled, caring environment.  We are committed to preparing our students to be lifelong learners who contribute to their communities through their service to others.  We are a school community that recognizes and services the individuality of each student.  Our teachers and staff are noted for their creativity and concern for students.  They welcome your interest in your child.  We have increased our extracurricular activities to provide your child the opportunity to discover and develop talents in chess, chorus, karate, drama, mock trial, and environmental issues.  We continue to offer middle school sports; cross country, soccer, basketball, track and baseball.  Our “Holy Family Chapel” is a source of comfort for our school community.

In order to obtain accurate enrollment figures that we must have to appropriately plan and prepare for the upcoming year, we have expanded the options for payments of the required deposit and increased your options for tuition payments.

A non-refundable deposit of $500.00, for each student, may be paid in two installments of $250.00 due on February 13, 2009 and May 1, 2009.  We will accept MasterCard, Visa and Discover card payments for deposits.

The tuition increase this year will be $250.00, pending Diocesan approval.  We will also accept credit card payments for full year, half year and new this year quarter year tuition payments.  There will be a $150.00 athletic fee per sport for Middle School and a $70.00 materials fee for pre-school (to be billed separately).

Enclosed are the Re-Registration Form/Enrollment Contract, FACTS Grant and Aid Assessment Form and the Parish Financial Aid Form for the 2009-2010 school year.  The Re-Registration/Enrollment Contract and first deposit payment should be returned to Monsignor Clarke School no later than Friday, February 13, 2009. 

Anyone requesting any financial aid (i. e. Monsignor Clarke aid, Diocesan aid and/or Parish aid) must fill out the FACTS Grant Aid Assessment Form.  It is recommended that it be returned as soon as possible but it must be postmarked by February 13, 2009.  This form is sent directly to FACTS.  If you are applying for Parish aid, the Parish Financial Form must also be filled out and sent directly to your Pastor.

HEALTH INFORMATION:

Currently enrolled students who are entering kindergarten need to submit the RI SCHOOL PHYSICAL FORM, including updated information on immunizations and evidence of lead screening and vision screening, before the start of the school year.

Currently enrolled students entering grade 7 must also complete the RI SCHOOL PHYSICAL FORM, including updated information on immunizations, before the start of the school year.

All students, regardless of grade, should submit a Student Health History Form before the start of the school year.

Medication Policy:

All medication should be supplied by the parent/guardian.  Medication Forms are to be submitted prior to the dispensing of any medications in school.  Prescription medication requires the signature of the doctor and parent or guardian.  Non prescription medication requires the parent/guardian signature.
